Records Retention Policy
HEALTH AND SAFETY Telephone - 915-5433 The Health and Safety Office is the official repository for documents shown below. The department is responsible for retaining the official copy of a record and serve as the department responsible for the development, maintenance, retention and destruction of records. Records are retained as specified by UM, IHL, State, Federal or outside entity guidelines. Departmental records are retained at the discretion of the department but are encouraged to follow the guidelines outlined in order to reduce labor and save space. |

The following files/records are retained on a permanent basis: Annual Report of Radiation Exposure of Personnel, Certificates of Disposal of Hazardous Waste, Certificates of Recycling Hazardous Materials, Information Files on Authorized Users of Radiation Generating Devices or Radioactive Materials, Lab Pack Drum Inventories, Land Disposal Restriction Notification Forms (LDR's), Off Spec Notification/Resolution Forms, Radioactive Materials, Sealed Sources Inventories, Termination of Authorization Notices, TLD Dosimetry Reports, Toxicity Characteristic Leaching Potential (TCLP) Results, Uniform Hazardous Waste Manifests |
